Minimum Qualifications

· Bachelor Degree in Human Services or related field

· Two years of experience working with Adult I/DD population

· Experience with PCP/ISP writing and developing short range goals and objectives

· Supervisory and staffing experience

· CPI, CPR/First Aide and NCSNAP Certified

· State recognized Qualified Professional with experience in coordinating and maintaining a case load of 20 +adults with intellectual developmental disabilities

· General Statute 122C-80 Criminal History Record consent is required as well as fingerprint check if the applicant has been a resident of NC for less than 5 years

OTHER RELEVANT KNOWLEDGE, SKILLS AND ABILITIES:

· Familiarity with NC State and Medicaid guidelines

· Experience with Accreditation Organization, ensure meeting and or exceeding accreditation standards

· Experience with Managed Care Organization (MCO) and community providers

 

Preferred Qualifications

Working in Community College environment or equivalent.

Position Description

GENERAL FUNCTION

Under general supervision, the purpose of the position is to expand and improve service that is provided through the Brunswick Interagency Program.  This position assists with fostering strong relationships with partners or collaborating agency/agencies by enhancing the client experience.  By improving responsiveness and streamline the assistance process for the family and coordinate all supportive services.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES FOR ENHANCED SERVICES COORDINATOR

· Provides routine supervision to day support/paraprofessional staff.

· Serves as the Managed Care Organization liaison in order to monitor NC DHHS and NC DMA           policies and Trillium contract.

· Responsible for the development of person centered plans, updates, and revisions

· Oversees the provisions of services, by receiving authorizations for services, maintaining       ongoing documentation, preparing claims and billing for all IDD services

· Assists with enrollment of new programs participants

· Aids program in maintaining national accreditation

· Ability to multi-task and prioritize daily needs of students, staff and program

· Proven record of problem solving and making sound decisions under pressure

· Participates actively in the Quality Assurance and Quality Improvement committees, processes and project

· Engages in advocacy efforts on behalf of students

· Takes part in public speaking, public awareness initiatives, and special events

· Establishes and maintains rapport and communication with students, families, and community partners

· Ensures quality day support services and documentation for all IDD services
