Minimum Qualifications
A Bachelor’s degree is required, in the area of Social Work, Criminal Justice, Psychology, Human Services, or a related field. The degree must be from a College or University, accredited by one of the six accreditation bodies in the United States recognized by the Department of Education (DOE). A minimum of three years of verifiable case management experience is required. Individuals who have worked with or provided services for Justice Involved Individuals are preferred. Required to have a valid driver’s license and properly registered and insured motor vehicle that meets the minimum level of coverage

Human Interaction: Requires the ability to provide guidance, assistance, and/or interpretation to others regarding the application of procedures and standards to specific situations.
Equipment, Machinery, Tools, and Materials Utilization: Requires the ability to operate and control the actions of equipment, machinery, tools and/or materials requiring complex and rapid adjustments.
Verbal Aptitude: Requires the ability to utilize a wide variety of reference, descriptive, and/or advisory data and information.
Mathematical Aptitude: Requires the ability to perform addition, subtraction, multiplication, and division; the ability to calculate decimals and percentages; the ability to utilize principles of fractions; and the ability to interpret graphs.
Functional Reasoning: Requires the ability to carry out instructions furnished in written, oral, or diagrammatic form. Involves semi-routine standardized work with some latitude for independent judgment concerning choices of action.
Situational Reasoning: Requires the ability to exercise judgment, decisiveness and creativity in situations involving the evaluation of information against sensory, judgmental, or subjective criteria, as opposed to that which is clearly measurable or verifiable.
Physical Ability: Tasks require the ability to exert light physical effort in sedentary to light work, but which may involve some lifting, carrying, pushing and/or pulling of objects and materials of light weight (5-10 pounds). Tasks may involve extended periods of time at a keyboard or work station.
Dexterity: Requires the ability to perform semi-skilled coordinated movements, such as those required to operate office equipment.
Sensory Requirements: Some tasks require the ability to perceive and discriminate visual cues or signals. Some tasks require the ability to communicate orally.
Environmental Factors: Essential functions are regularly performed without exposure to adverse environmental conditions.
Position Description
Serves as the “point of contact” for supportive services and ensure a successful delivery of reentry services to participants.
Assists in the development of partnership agreements to allow justice involved individuals to be referred to service providers.
Ensures successful delivery and managing of the case management and referral process.
Helps to coordinate and facilitate (in conjunction with the executive committee) local reentry council meetings and provide public education.
Works with the PCC Reentry Programs Coordinator to coordinate the development and implementation of an appropriate public outreach and education strategies to inform the community of the mission, goals, and strategies of the LRC.
Generates reports and facilitates the review of data to help the LRC to make appropriate decisions regarding budget, strategy, etc.
Conducts assessment of the program to help the LRC to identify strengths and weaknesses on a yearly basis.
Communicates needs and services that are not readily accessible or available within the council partners.
Serves as an ex-officio member of the Executive Committee; Participates in and attends all sub-committee meetings along with the other Case Manager.
Oversees the submission of accurate and timely records for payment and reimbursement; Makes sure that required data submission is on time and accurate.
Maintains and establishes a relationship with all vendors working with the LRC by making sure MOU/MOA’s are in place and up-to-date and are being properly executive.
Works with the LRC to identify resources that might enhance the delivery of services to participants beyond that which the grant can provide.
Ensures that the intake process and the servicing of participants meet the standards and requirements set forth by the SOP and DPS Reentry Guidelines.
Oversees the work of the Case Manager to assure that it is accurate and up-to-date.
Works with Pitt County Probation and Parole Office and other stakeholder entities to ensure they are properly served through the referral process.
Makes sure that the physical location and place of operation is functioning at optimal efficiency.
Attends all meetings as requested by State DPS Reentry and Pitt Community College, as well as trainings.
Provides scheduling, supervision, and evaluation of Case Manager’s performance.
Assists in coordination of pre-release orientations; Reviews all participant files and Re-Entry activities for the purpose of compliance.
Ensures the accountability of Re-Entry participants and project success.
Reports regularly to Executive Committee on progress of the program.
Works with Council Partners to develop long-term plans and sustainability.
Supervises, assigns, and manages Interns and Volunteers.
Must be able to adequately perform all of the skills and duties required for the regular Case Manager role (see Reentry Case Manager JD). Ability to exercise discretion when dealing with matters of significance and consequences that affect the participants and the agency.
